We accept the following INSURANCES. If your insurer is not listed, contact us for the most up-to-date information. We verify patient benefits prior to the first visit and communicate that information to you. We also accept self-pay clients.

Monday-Thursday 8am-6:30pm

Friday 8am-5pm

In most cases you do not.

Medicare and Workers Compensation require a referral.

Your Insurance card (if using) and your ID.

45 minutes to 1 hour

A full evaluation with light treatment and home exercise program or splint if needed.

If you don’t take insurance we do take Self Pay.

A referral from your doctor and an authorization from the adjuster are required to start treatment.

We do accept walk-ins.

We can accommodate this upon request.
